Your privacy is important to us.
Haliburton County Paramedic Service and our staff ensure that your health information is treated with respect and sensitivity and that your privacy is protected.
All documents containing personal health information are secured and access is limited.
Access to your personal health information is limited to those who are involved in your emergency medical care.
Paramedics collect personal health information about you directly from you or from people acting on your behalf during an emergency. We may use and disclose your health information for:
- emergency treatment
- OHIP Payment
- quality improvement
- statistics
- legal requirements
How to access your Personal Health Information
You can access a copy of your own Ambulance Call Report. You must contact the office during regular business hours (Monday – Friday 8:30 a.m. to 4:30 p.m.). You will be required to complete a Release of Information Form and show proof of identity.
