Requesting your ACR
Please call the office during business hours (Monday – Friday 8:30 a.m. to 4:30 p.m.). We will need to know your name and the location and date of the event. You must complete a Release of Information form and show proof of identity. Please email our Paramedic Service for a copy of the form
Fees
Legal firms requesting an ACR for a client must pay a fee of $75.00. Please submit the Release of Information form with a cheque payable to The County of Haliburton to the following address:
